Please get these documents ready to move forward with your application:

1. The child’s ID

This can be a full birth certificate, a passport, a letter from your GP, or a letter from a Hackney Ark professional. 

2. Proof of Hackney address

This must show the same address on your DLA award letter.
It can include the following bills: gas, water, electric, satellite television bill, landline phone bill, or your council tax statement, or a bank statement.

3. Your child's DLA or PIP award letter

This shoudl be for the middle or high rate (DLA) or enhanced rate (PIP).
